Support for care leavers with disabilities

Birmingham Children's Trust Disabled Children's Social Care (DCSC) team support young people with disabilities, including discussions about your preparation for adulthood.

If your needs mean that you will qualify for adult care, a referral will be made by the time you are 17, with a handover to adult care around your 18th birthday.

If you do not qualify for adult care you will still be able to access a Personal Advisor (PA).
